You may be ready to leave hospital but would find it hard to manage at home without support. The ward staff will talk to you about your needs and what matters to you. They may refer you to our Social Care team who will help you to decide what support you need from us, alongside any help you may receive from your carer or family.
Our aim is for you to stay in your own home as long as possible, therefore when you are discharged from hospital we may arrange a temporary reablement support package. We will help you to achieve a safe and timely discharge and once you are home, help you towards recovery and independence.
If you need some equipment or adaptations to help you to move around safely at home, we can help to arrange for these items to be delivered or fitted in your house.
In the event that you are not able to return home, we will talk to you about the options available to you. You may need to consider living in a different setting, such as Extra Care Housing or a residential home. This will depend on your needs at the time.
