We will be carrying out essential website maintenance in the afternoon which will affect some functionality. We apologise in advance for any inconvenience the work may cause and will do all we can to keep disruption to an absolute minimum.

You may be eligible for help if you:

  • Are over 18 years old
  • Live in Conwy County Borough
  • Need practical help because you are ill or frail, have sight or hearing loss or have physical or learning disabilities

If you believe that you or someone you care for needs help with daily living or improving wellbeing you should contact our team to assess your needs.

Your assessment

To work out how much help you need we will talk to you to find out what matters to you, and what support may help you to live independently in your own home.

The assessment will take place over the phone or in your own home.  You can have a friend, relative or carer with you.  If you are in hospital we will come and talk to you before you are discharged. You can tell us what help you might need at home whilst you are recovering.

The assessment is a two-way conversation. We will ask you to tell us about yourself, and how you think we can help and support you.  We might ask the opinion of other professionals who know and work with you, like your doctor, but we will only do this with your permission.

We will also take into account the needs of your family or carer.

What happens next?

Once we have assessed your practical needs we can put together a short-term care package to help you regain or improve your independence.  We call this ‘reablement’.  If we agree that you have on-going care needs after the period of reablement, we can help you to arrange this.

We will continue to assess your needs to see if there are any tasks that you are still struggling with and need support for.

You will have a financial assessment to find out if you have to pay anything towards the cost of your ongoing support needs.  If you are unhappy with the outcome of the assessment you can ask us to look at your situation again with you.  If you are still unhappy, you can contact us to make a complaint or comment.

If you don’t qualify for on-going help from us we will give you information and advice on other services that are available to help you continue to live independently.
