Personal Advisor Team

We work with young people over the age of 18 as they make the transition from being in the care of the local authority to independence.

We will continue to provide support up to the age of 21, or up to the age of 25 if both the young person and council agree. This will be recorded in the Pathway Plan.

The Personal Advisors teams provide statutory duties and responsibilities to care leavers which includes offering advice and support with education, training and employment, accommodation and general welfare.

Accessing Support

If you are under the age of 18 you will be referred to the Personal Advisor Team by your Social Worker in the Pathways Team.

If you have been closed to the service after the age of 21 and would like to reconnect with us, please contact our Single Point of Access Team on 0300 456 1111 and ask to be referred to the Personal Advisor Team.
