Dementia – Accessing the right support, at the right time: North Wales Memory Support Pathway

Are you worried about your memory? You’re not alone. An estimated 11,900 people live with dementia in North Wales, and around 67,000 unpaid carers support loved ones, according to the 2021 Census.
The North Wales Memory Support Service offers tailored support for people affected by dementia. This includes individuals who are concerned about memory issues, those living with dementia at home or in hospital, and their friends and family.
The service supported 4,700 people in 2023/24, over 1,200 more compared to the previous year. Support is available at every stage of the dementia journey—before, during, and after diagnosis. A collaboration between the six Local Authorities, Betsi Cadwaladr University Health Board, Alzheimer’s Society, Carers Trust North Wales, Carers Outreach, and NEWCIS ensures advice and guidance is personalised to meet individual needs.
You can access support by visiting or contacting one of six Dementia Centres. There is one in each county in North Wales. They are located in Llangefni, Bangor, Colwyn Bay, Denbigh, Holywell, and Wrexham. No referral from a GP or professional is necessary, though professionals can also make contact on the individual’s behalf.

The Dementia Centres provide:
- Information and advice
- Activities and Catch ups
- Emotional and peer support
- Guest speakers and health clinics
Cllr Penny Andow, Conwy County Borough Council’s Cabinet Member for Integrated Adult and Community Services, said: “The support provided by the Memory Support Pathway can help change the lives of people with this disease. It’s part of the Partnership’s commitment to recognise the need to support people to live well with dementia and to help them remain a part of their local community.”
This is one of many services funded by the North Wales Regional Partnership Board (RPB), who support the delivery of a variety of health and social care initiatives for people and communities across the region.
