Data Protection - Applying for your data

To apply for your data, you must do the following:

• The Council has created a Subject Access Request form (Word, 183Kb) to assist you in providing as much information as necessary to enable your request to be dealt with as efficiently and effectively as possible.
  
• Give us your full name and address.
• Provide proof of your identity (original documents only) i.e passport, driving licences, birth certificate
• Provide proof of your address i.e household bill, bank statement.
