The Pathway
The Pathway's role is to find the most suitable support service for your need and send the referral on to the provider of the service for them to make contact with you and arrange support. If you or someone you know may be in need of Housing Related Support then you can:
- ask your social worker or health professional to contact us
- ask a friend or a member of your family to contact us
- contact us yourself by email or by completing the Pathway referral form (attached below) and returning it to SPPathway@conwy.gov.uk or by post to:
