  • Trainer: North Wales Learning Disability Transformation Team
  • Targeted services: Business & Transformation, Older People & HSW, Community Wellbeing, Quality Standards, Disability Team, Vulnerable People Team, Family Support & Intervention, Youth Justice Service, Children Looked After, Foster Carers, Commissioned Services (PIVs)
  • Target group: Staff who directly support people with learning disabilities, Managers of support and care services, statutory or third sector Professionals, parent carers, advocates, housing support professionals and other interested individuals who wish to learn more about the concept, principles and methods of using the tools

Course aims and objectives

Give a detailed overview in understanding and using practical person centred core planning tools. Including; One page profiles, Working/ Not working, Communication charts, Decision making agreements, Learning logs, Staff matching, 4 plus one, Relationship circles and the creation of a person centred Support Plan.

The session will look at: Understanding the essential and preferred needs and wishes of citizens and creating a person centred support plan.
